ABDM Consent Declaration

I hereby declare that:

1.I am voluntarily sharing my Aadhaar details and demographic information for the sole purpose of creating ABHA ID by National Health Authority (NHA).
2.I am aware that my ABHA number can be used to fetch my health details by entities working under National Digital Health Ecosystem (NDHE) by facilities registered with ABDM, as and when required by me for availing health care services.
3.I am aware that my personal identifiable information (Name, Address, Age, Date of Birth, Gender, and Photograph) can be accessed using my ABHA ID.
4.I authorize NHA to use my Aadhaar number for performing Aadhaar-based authentication with UIDAI as per the Aadhaar (Targeted Delivery of Financial and other Subsidies, Benefits and Services) Act, 2016.
5.I understand that UIDAI will share my e-KYC with NHA upon successful authentication.
6.I am aware that there are other options for registration; however, I consciously choose Aadhaar for creation of my ABHA ID.
7.I reserve the right to revoke this consent at any point of time under the provisions of Aadhaar Act and Regulations.
