Best PHR Apps in India 2026: Don’t Just Store Your Health Records—Understand Them

A 2026 comparison of India's PHR apps — ABHA, ExaHealth, DigiLocker, Eka Care and more — on ABDM integration, family profiles, AI report reading and independently verified security.

An Indian family of four sitting together on a sofa, smiling while looking at a health app on a smartphone

India crossed a real milestone in 2026: over 100 crore health records are now linked to Ayushman Bharat Health Accounts (ABHA), and more than 93.95 crore ABHA IDs have been created nationwide. That's the ABDM ecosystem working at scale — but ABHA itself is just an address, not a place to actually manage your family's reports. That's what a PHR (Personal Health Record) app does. To help you choose, we evaluated the best PHR apps in India in 2026 — comparing what each one is genuinely good at, where it falls short, and how to pick the right one for your family.

What is a PHR app, and why does it matter right now?

A Personal Health Record (PHR) app is software that lets you store, organize and share your own medical records — lab reports, prescriptions, discharge summaries, vaccination certificates and scans — under your control. In India, most credible PHR apps now plug into the Ayushman Bharat Digital Mission (ABDM), the government's digital health backbone, using your ABHA (Ayushman Bharat Health Account) as the identifier that lets hospitals, labs and apps exchange your records with your consent.

The numbers explain why this matters. As of July 2026, ABDM had:

    • 93.95 crore ABHA numbers created

    • 105 crore+ digital health records linked — up from 50 crore just fifteen months earlier

    • 450+ public and private health-tech platforms integrated into the ecosystem

    • 5.33 lakh health facilities and 9.85 lakh healthcare professionals registered on national digital health registries

That's roughly 10 crore new records being linked every two to three months. The infrastructure exists — the real question for most families isn't "is ABDM real," it's "which app do I actually use to make sense of my own records."

Note: This article compares record-management apps, not doctors or treatments. Nothing here is medical advice — always consult a qualified clinician for diagnosis or treatment decisions.

    How we compared these apps

Every "best PHR app" list makes claims. We narrowed ours to seven criteria that actually change whether an app is useful day-to-day for an Indian family, and we've marked a comparison cell blank rather than guess wherever a provider hasn't published the detail themselves:

    1. ABHA/ABDM integration depth — can it pull records from ABDM-linked hospitals and labs, or just create an ABHA number?

    2. Family profiles — can one account hold parents, spouse and children, or does everyone need a separate login?

    3. Photo/paper record capture — can you photograph an old paper report and have it filed automatically, or only pull digital records?

    4. AI report explanation — does it explain what a lab value means, or just store the PDF?

    5. Data portability — do your records stay yours if you stop using the app?

    6. Security certification — independently verifiable certifications, not just marketing claims

    7. Price — what's genuinely free versus what needs a paid tier

Security certifications and pricing are covered in the write-up below each row, and in the "Is your health data actually safe" section further down. This table was last checked against each provider's own site in 2026.

    1. ABHA app — the official government reference app

The ABHA app, built by the National Health Authority, is the official reference app most people use first — it's how you generate your 14-digit ABHA number. It lets you create a memorable ABHA Address (yourname@abdm), pull CoWIN vaccination certificates, and view records linked from ABDM-enrolled facilities, with consent, in one place. It's free, and government-operated by the National Health Authority.

    2. ExaHealth — an AI-first PHR built around a health score, not just storage

ExaHealth is an ABDM-certified PHR & Health Locker with a different starting premise from the rest of this list: instead of just storing documents, it reads and structures every report with AI, then turns that into something a person can act on.

What that looks like in practice:

    • Records that file themselves — a photo, a WhatsApp forward, a Gmail attachment or an ABDM pull all get OCR'd, read and auto-filed by family member, provider and date — not just stored as a raw PDF.

    • Lab values explained in plain language, checked against curated reference ranges (WHO, ICMR, AHA, KDIGO, ATA) with a clear "no assessment" label rather than a guess when a value can't be classified.

    • Ask Exa, an AI assistant that answers questions about your own records in plain language — in the app or on WhatsApp — including medicine-interaction checks against everything you're currently taking.

    • Family profiles — each family member gets their own records, so parents' reports live in the same place as yours.

    • A one-page visit report you can carry into any consultation: history, current medicines, recent results and what's changed, generated automatically.

    • Security that's independently certified, not just claimed: ABDM-certified PHR & Health Locker, ISO/IEC 27001:2022 (information security), ISO 9001:2015 (quality), UKAS-accredited and IAF-recognised.

Pricing is straightforward: free forever for the basics (50 documents, 4 family profiles, first 10 AI analyses), or ₹2,999/year (about ₹8/day) for unlimited documents, all 14 Health Score dimensions and unlimited AI analysis on the Plus tier.

    3. DigiLocker — for Aadhaar and government documents, plus health records

DigiLocker's health-records layer, launched via its ABDM Level-2 integration, sits alongside the Aadhaar, driving licence and other government documents most Indians already keep there. You can scan and upload old paper records, link ABDM facility records to your ABHA, and share selected documents with ABDM-registered providers. It's free, and government-operated by MeitY.

    4. Eka Care — India's largest consumer PHR app by user count

Eka Care is, by most public accounts, the biggest consumer PHR platform in India today, reportedly used by 30 million+ people. Its strength is ABHA auto-linking — when you visit an ABDM-enrolled provider, records tend to show up in Eka Care without extra steps — plus a large affiliated doctor network for teleconsultation and an AI HealthBot for quick health questions. It supports photo capture for lab reports and family profiles for managing more than one person's records, with paid consultations available alongside the free core app.

    5. Health-e (CareCircle) — family-first health locker

Health-e (CareCircle) consolidates a family's records into one ABDM-aligned health locker, with its own AI assistant for interpreting reports and support for multiple family profiles under one account.

    6. MyDigiRecords — AI vitals tracking with ABDM certification

MyDigiRecords pairs ABDM certification with an AI feature it calls SmartVitals, for facial-scan vitals tracking, and supports up to five family profiles.

Also worth knowing: Practo, Paytm's ABHA Health Locker, Docprime and a handful of other apps also offer ABHA-linked record storage as part of a broader booking or payments product — worth a look if you already use the parent app for something else.

How to choose the right PHR app for your family

There's no single "best" app — there's the best fit for what you're actually trying to solve:

    • You just need an ABHA number and nothing else: the official ABHA app is enough. Don't install a second app you won't use.

    • You're managing health records for parents, a spouse and kids: look for family profiles and photo/paper scanning — most of a typical Indian family's older records are still on paper, not sitting in an ABDM-linked hospital system yet. ExaHealth includes family profiles with photo, WhatsApp and Gmail capture built in.

    • You or a family member is managing a chronic condition (diabetes, hypertension, thyroid): look for an app that explains lab trends over time and flags drug interactions, not just stores PDFs. ExaHealth's AI reads every report against clinical reference ranges and checks new medicines against everything you're already taking.

    • You're already deep in one government app ecosystem (DigiLocker for other documents): DigiLocker itself is just storage — an AI-first app like ExaHealth can sit alongside it and add the trend tracking, family view and plain-language report explanations DigiLocker doesn't do.

    • Security matters as much as features to you: check for independently verifiable certifications rather than only reading a vendor's own privacy-policy language. ExaHealth is ABDM-certified and independently audited to ISO/IEC 27001:2022 and ISO 9001:2015.

For a family managing more than one person's health, ExaHealth is built to cover all of the above in one app.

Family profiles matter most for households managing more than one person's records.

Is your health data actually safe in these apps? The ABDM basics

Every credible PHR app in this list operates inside the same consent architecture, so it's worth understanding once rather than re-reading on every provider's privacy page:

    • Consent-first by design. An ABDM-linked hospital or lab (a "Health Information Provider," or HIP) cannot hand your records to an app (a "Health Information User," or HIU) without you approving a specific consent request — for a specific date range and purpose.

    • You can revoke access. Consent isn't permanent; you can withdraw it for any provider at any time.

    • Records are encrypted in transit under ABDM's data-flow protocol, using a Diffie-Hellman key exchange between the sending and receiving systems — the record itself is encrypted before it leaves the source.

    • Certification isn't automatic. An app being "ABDM-integrated" and an app being independently security-audited (ISO/IEC 27001, for instance) are two different claims — the first is about interoperability, the second is about how carefully your data is actually protected once it's in the app's hands. Worth checking both before you trust an app with a parent's full medical history.

ABDM security rests on consent architecture, not just a certification badge.

    Frequently asked questions

Is the ABHA app enough, or do I need a separate PHR app?

The ABHA app is enough if all you need is your ABHA number and to view records from ABDM-linked providers one person at a time. Most families outgrow it quickly — there's no way to manage a parent's or child's records alongside your own in one login, and it can't digitise the paper reports most Indian households still have piles of.

Can PHR apps store my whole family's health records in one account?

Some can, some can't. The official ABHA app and DigiLocker are built around one person per login. Apps like ExaHealth, Eka Care and Health-e are built around family profiles from the start, letting parents, a spouse and children live under one account with their own separate records.

Are PHR apps in India free?

The government apps (ABHA, DigiLocker) are free. Most private apps offer a genuinely usable free tier and charge for higher limits — more stored documents, more AI usage, more family profiles. ExaHealth, for example, is free for up to 50 documents and 4 family members, with a ₹2,999/year Plus tier for unlimited use.

Is my data shared with my employer or insurer through ABHA?

Not without your explicit consent. ABDM's architecture requires a specific consent approval before any provider or app can access your records — an employer or insurer isn't automatically able to see them just because you have an ABHA number.

Can I switch PHR apps without losing my records?

If the app is ABDM-linked, your underlying records stay tied to your ABHA, not locked inside one app — you can generally re-link them elsewhere. Records you've manually uploaded or scanned (rather than pulled via ABDM) may not transfer automatically, so it's worth checking an app's export options before you commit to it long-term.

Do PHR apps replace my hospital's medical records?

No. A PHR app is your personal copy for reference and continuity of care — it doesn't replace the legal medical record a hospital or lab is required to maintain on its own systems.
